There are a few things that should always be included in a note for a medical excuse.

If you are opening your own private practice, or maybe you work in a doctor’s office, you are familiar with your patients asking you for a doctor’s note. A doctor’s excuse note is usually required by an employer when an employee is out ill, especially if they are out or are going to be out for an extended period of time.
Creating a template for your office to use is time-consuming and a waste of your time. We provide free doctors note templates for you to fill out and hand to a patient! Creating your own doctor’s excuse template for your medical exemption note also means that it will be unique to your practice and no other practice in the area will have it. A doctor’s note doesn’t have to be perfect, but it should easily communicate with an employer that their employee was seen.
There isn’t a set template for these notes, so a practice may use whatever format they wish. Unfortunately, this means that you may have a lot of work ahead of you to design your own template.

This question is a tricky one. The short answer is, “yes”. An employer may call the number on your work excuse and verify whether you were given a note. However, this is all they can do. Any further questioning would lead to a HIPAA violation, meaning that the employee who gave out the information will be in violation of the law. Secondly, not only could you get into legal trouble for faking a note, you could lose your job and livelihood by doing so!
